Female, white, married, housewife; born Jan [8?] 1883 in Illinois; aged 35 years 10 months 13 [?] days. Father “Finley Ellingwood, M.D.”; mother Jenn[ie] S. “Ellis” [sic, Elliott], born New York. Informant Finley Ellingwood, 2209 Central [Street, Evanston]. Buried at Memorial Park, 23 Nov 1918; undertaker Wm. A. Duffy.
